Country Health Connect is participating in a web-based active vaccine safety surveillance program called Vaxtracker. We currently use this program during our public immunisation sessions and are expanding it to include the Year 7 school immunisation program this year, with Year 10 students to be included from 2027.
Vaxtracker monitors any side effects following vaccination through a short survey. The program helps ensure vaccines remain safe by identifying any higher-than-expected or unusual rates of adverse events. Responses contribute to the national vaccine safety surveillance program.
After their child has been vaccinated, parents or caregivers can register by scanning the QR code on the Vaxtracker poster. A few days later, they will receive a short survey via SMS or email asking about any reactions their child may have experienced following vaccination.
The survey is completed and submitted by the parent or caregiver. All responses received by AusVaxSafety are de-identified to protect participants’ privacy.

During our school visits, each student receives a Personal Immunisation Record leaflet following vaccination. We will also provide each student with the Vaxtracker poster containing the QR code.
